- Family Members 12 and Older Eligible for On-Campus COVID-19 Vaccine
Dear Colleague,
Pfizer-BioNTech has received Emergency Use Authorization that expands access for the vaccine to protect against COVID-19 to those who are 12–15 years old. Therefore, we are pleased to share that we will add an additional vaccination day for employees who may want to bring family members including children 12–17 years of age to campus for the vaccination.
The first dose will be on Monday, May 24, and the second dose, required 21 days later, will be on Monday, June 14. Participants must return to campus to receive the second dose. Appointments will be available from 11 a.m. to 5 p.m. to include a few after-school hours. We will administer these vaccines to your family member at no charge. Please review each section below for more information. The vaccine distribution location is Taffner Field House on the Queens, NY, campus—family members of employees from all St. John’s University campuses are welcome!
Who Is Eligible for the COVID-19 Vaccine Provided on Campus?
Family members of all employees of St. John’s University who are 12 years of age or older.
Appointments to receive the vaccine on campus are required.
Pfizer Requires Two Doses of the Vaccine.
St. John’s will distribute the Pfizer-BioNTech vaccine. This vaccine requires two doses, spaced 21 days apart. You must return to St. John’s to receive your second dose.
If you cannot return to campus for the second dose, do not make an appointment with us. You must be available to receive both doses on the St. John’s Queens campus.
How Do I Sign up to Receive a COVID-19 Vaccine on Campus?
You are required to complete the following items to receive an appointment. Please be sure each family member completes all forms and brings the paperwork with them to the appointment to ensure a quick and easy process.
Step 1. Make an Appointment.
You will need 30 minutes to complete the vaccination process. Appointments are on a first-come, first-serve basis. The appointment for the second dose will be scheduled on site after you receive the first dose.
St. John’s employees will make the appointment for their family member(s) by completing the COVID-19 Vaccine Registration Form for each person.
Step 2. Complete the Required Consent Forms.
Before arriving for your vaccination appointment, you must complete the three forms below and have your family member bring the paperwork with them to the appointment.
Please follow these steps to complete the required forms:
- New York State COVID-19 Vaccine Form
- Please note this will bring you to an external site hosted by New York State. Complete the form and print it to bring it with you.
- St. John’s COVID-19 Vaccine Consent Form
- This is the consent form that is required by New York State and authorizes St. John’s to administer the COVID-19 vaccine.
- For Those Under 18 Years Old: Parent/Guardian St. John’s COVID-19 Vaccine Consent Form
- If your family member is 16 or 17 years old, the parent/guardian will need to complete this form also and bring it to the appointment.
Please make sure you have completed all three forms. Remember to bring the completed forms with you to your appointment.
Step 3. Plan 30 Minutes for Your Appointment.
Please arrive at your appointment a few minutes early to check in. The appointment will take approximately 30 minutes total—roughly 15 minutes for processing the required forms and receiving the vaccine and 15 minutes for postvaccine observation. This observation period is a required precaution to make sure you are feeling well and not experiencing side effects from the vaccine.
How Much Does the COVID-19 Vaccine Cost?
The COVID-19 vaccine is free. We will not charge any administration fees.

Bring Your ID.
St. John’s employees should have their St. John’s StormCard when they accompany a family member(s) to Taffner Field House.
What to Wear
Please wear a short-sleeve shirt or a shirt that allows easy access to your upper arm. The vaccine injection site will be approximately three inches below the shoulder bone.
