What is the purpose of this form?
St. John’s University is committed to the safety and security of our students and community. The purpose of this form is to notify St. John's University that an incident of sexual misconduct has occurred. This form allows SJU to respond with support resources and address inappropriate behavior.
What is considered sexual misconduct?
Many different incidents fall under the umbrella term of sexual misconduct, including sexual harassment, sexual assault, dating/domestic violence and stalking. The relevant definitions are listed in the University’s Sexual Misconduct Policy .
How do I know if the harm I experienced is sexual misconduct?
It is okay if you do not know how to define what happened and/or don’t have all the details of the incident.

When will I get a response?
Please note that incidents submitted via this form may not be reviewed until the following business day. This form should NOT be used in an emergency.

What happens after I submit this form?
The contents of this form will be submitted to Danielle Haynes, Director of Equal Opportunity, Compliance, and the Title IX Coordinator, who will review the information. This form does not automatically initiate a Student Conduct Process. It provides you with an opportunity to inform the University of your experience so that we can offer support and explore options for moving forward, which may include an investigation and hearing. If an investigation and hearing reveal that a SJU student or employee violated the University’s Sexual Misconduct Policy , then appropriate sanctions or corrective action will be imposed.
What if I’ve also reported my experience to the police?
The University is required by federal law to respond to any disclosure of sexual misconduct and the information you submit may be investigated by the University independent of any criminal investigation.
If there is legal action related to your experience, the prosecutor, the perpetrator, and others involved in a criminal investigation might be able to read what you write here by legally compelling you, the University, or anyone else with access to this form to provide it. If that happens, anything you write down here might be used against you. We suggest that you only write down what you are certain of and are okay sharing if others were to access this document. If you are uncomfortable answering any part of this form, skip it. Leave as many questions as you’d like blank.
